Beginning January 1, 2019, the Pennsylvania Department of Human Services will institute its new Community HealthChoices (CHC) Program in Southeast Pennsylvania. This program will affect all nursing home residents on Medicaid.
CHC will use managed care plans to coordinate nursing home care for each Medicaid recipient. Care plans will be delivered through one of the following: AmeriHealth Caritas Pennsylvania (Keystone First), PA Health and Wellness, and University of Pittsburgh Medical Center Community HealthChoices. While services and Medicaid benefits currently being received will not change, all Medicaid recipients will need to choose and enroll in one of the three managed care plans. You will need to enroll promptly following the DHS notice requirements, or you will be randomly enrolled prior to January 1.
In the next couple of weeks, more information and an enrollment packet will be sent from the PA Department of Human Services to all Medicaid recipients. We have begun educating residents about the change and have met with members of the Family Task Force. Additionally, two family information sessions will be held here at the Center for any family member interested in learning more:
