The Madlyn and Leonard Abramson Center for Jewish Life, a provider of senior services for over 140 years, announced today a partnership with Intel-GE Care Innovations ™, a joint venture between Intel Corporation and GE Healthcare to study the effectiveness of leveraging remote patient monitoring (RPM) for patients in the home post discharge.
Patients diagnosed with Cardiac and Pulmonary Disease and/or CHF or COPD will use Health Harmony, Care Innovations’ RPM solution to enable patients, clinicians, care providers, and family caregivers to collaborate for better care of patients in their home.
“Health Harmony provides The Abramson Center a unique post-acute remote care model to position the Center to operate in future risk based payer models,” said Sean Slovenski, CEO of Care Innovations. “As the healthcare landscape continues to evolve and the move to accountability affects us all, the Abramson Center will be well positioned for success with their acute care referral sources.”
Patients will use Health Harmony daily in order to inform their care team at The Abramson Center on their progress. Health Harmony will be used to conduct health sessions, capture vital signs such as blood pressure, weight and oxygen levels, and other information needed for caregivers to manage care plans, promote medication adherence, and remain on top of patients’ changing health.
